Death in a dream is almost never to be taken literally and rarely concerns physical dying. In dream symbolism, death classically represents transformation, the end of a life chapter, and the beginning of something new.
The death of another person in a dream can symbolise the ending of a relationship or a shift in your perception of that person. One's own death is often a sign of profound personal change — the old dies so the new can emerge.
In many spiritual traditions death is seen as a transition, not an ending. Those who die in a lucid dream can use the experience as a ritual of transformation: what is "dying" in you and what is being reborn can be explored through direct questioning in the dream state. It does not predict an event, diagnose a condition, or reliably signal lucid dreaming.
For journaling, note the scene, your emotion, and what happened just before and after it. If this image recurs in your own dreams, you may choose it as a personal dream sign and then do a reality check; recurrence is your observation, not a fixed meaning.
